The death of a Filipino national should be reported to, and registered with, the Philippine Statistics Authority (PSA) through the Philippine embassy or consulate having jurisdiction over the locality where the event took place.

ONLY deaths that took place in the following states can be registered or reported at the Philippine Consulate General in New York: (1) Connecticut, (2) Delaware, (3) Maine, (4) Massachusetts, (5) New Hampshire, (6) New Jersey, (7) New York, (8) Pennsylvania, (9) Rhode Island, and (10) Vermont.

Consular Mortuary Certificates are issued when the human remains of a deceased Filipino or foreigner are to be transported to the Philippines.

Requirements for shipment of human remains depend upon whether the body has been embalmed or cremated.

Typed or printed legibly in black or blue ink. All forms must bear the original signature of the informant and should be notarized by any US notary public. Submit four (4) original forms.

Flight Details

 

 

The itinerary must include full flight details and mode of transportation. Submit four (4) copies.

Death Certificate issued by the State Department of Health

 

Submit one (1) original and three (3) photocopies.

 

Information Sheet

 

Must be duly accomplished and signed. Submit four (4) copies.

 

Proof of Filipino Citizenship of the Deceased

 

Submit one (1) original and three (3) photocopies.
Samples of proof of Filipino citizenship are: Philippine Passport, US Permanent Resident Card, Valid Visa or Work Permit; or Dual Citizenship papers (Identification Certificate and Approval Order)
 

Letter of Acceptance of Remains

 

Must be duly accomplished and signed. Submit four (4) copies.

 

 

 

Affidavit of Delayed Registration (Notarized)

Required if application is filed more than one (1) year after the occurrence.

Must be executed and signed by the informant and duly notarized by any US notary public. Submit four (4) originals signed and notarized.

 

 

Proof of Identity of the Deceased

 

Philippine Passport or US Passport (if non-Filipino. Submit four (4) copies.

 

 

 


Certificate of Cremation (for cremated Remains)

 

Submit one (1) original and three (3) photocopies.

Certificate of Cremation issued by the crematory

 

 

Cremation/Burial-Transit Permit

 

 

For cremated remains, the permit is usually a 4” x 8” card or paper permit. Submit one (1) original/CTC and three (3) photocopies.

 

 

Health Certificate (for human remains)

 

Issued by the State Department of Health stating that the deceased did not die of contagious disease and that the human remains do not pose any danger to public. Submit one (1) original and three (3) photocopies.

 

 

Notarized Mortuary Certificate (for human or cremated remains)

 

Must state that:
(a) the body was properly embalmed/cremated in accordance with internationally accepted practices;
(b) the body/ashes are in sanitary condition for overseas shipment, consistent with existing standards; and
(c) the casket/urn has been hermetically sealed and contains only the human remains or ashes of the deceased.

The wife should indicate her maiden surname as her last name.

The husband and wife should accomplish and sign the four (4) ROM applications. The forms must be duly notarized by any US notary public.

 

If Widow or Widower:

PSA Death Certificate

Submit one (1) original/CTC and (3) photocopies.

 

 

Marriage Contract or Certificate issued by US vital records

 

Submit one (1) original/CTC and (3) photocopies.

The marriage contract or certificate must bear information on the number of marriages contracted by both parties. If the number of marriages does not appear on the marriage contract/certificate, please submit Marriage Record/License as an additional requirement.

If Annulled

 

Amended PSA Marriage Certificate, with Annotation of Annulment

 

Submit one (1) original/CTC and (3) photocopies.

 

Proof of Filipino Citizenship of Applicant/s at the time of marriage

 

Submit one (1) original/CTC and (3) photocopies.

Samples of proof of Filipino citizenship are: US Naturalization Certificate, US Permanent Resident Card, Valid Visa or Work Permit; or Dual Citizenship papers (Identification Certificate and Approval Order)

If Foreign Spouse was previously married to another foreigner

 

 

Foreign Divorce Decree

 

Affidavit of Delayed Registration (Notarized)

 

 

Required if application is filed more than one (1) year after the marriage.


Must be executed and signed by both parties and duly notarized by any US notary public. Submit four (4) original signed and notarized affidavit.

 

 

If the Filipino Citizen was previously married to a foreigner and a divorce was validly obtained abroad by the alien spouse

*Foreign Divorce Decree Duly Recognized by the Philippine Court.

There is no divorce in the Philippines, but when a divorce is validly obtained abroad by an alien spouse from his or her Filipino spouse, the Filipino spouse shall have the capacity to remarry under Philippine law. However, the divorce obtained abroad must be passed upon judicially by a Philippine court, to prove its validity, before the Filipino spouse can remarry under Philippine law.

The decision of the Philippine Court shall become the basis for the annotation on Philippine-issued Civil Registry documents.

 The Consular Officer reserves the right to require additional documents from the informant or applicant.

2. The Consulate assesses the completeness of the emailed submission.

3. Mail the Report of Marriage (ROM) Application packet

Once emailed documents have been determined to be complete and in order by the Consular Officer, the informant mails to the Consulate the ROM Application packet (please see below the contents of the packet), taking note of the envelope’s tracking number. Address the mail to:

CONTENTS OF APPLICATION PACKET

 

 

a. Covering Letter

Include a covering letter (handwritten or typed), addressed to the Civil Registry Unit, indicating the consular service requested, name of person filing the application, including the person’s mobile number and email address.

b. Complete DOCUMENTARY REQUIREMENTS

Arrange documents in this order. Please refer below to the corresponding number of documents to be submitted.

c. Payment

Processing Fee: $25.00 (ROM); $25.00 (per Affidavit)

Payment should be in the form of money order or cashier’s check payable to the Philippine Consulate General. Do not send cash by mail. Personal checks are not accepted.

d. Return Envelope

A self-addressed pre-paid stamped envelope, preferably USPS Priority Mail, with tracking number, must be included when sending the application. Do not use metered stamps or P.O. Box Return Address. Do not use Fedex.

The Philippine Consulate General assumes no responsibility for any delay or loss in the mail, or while the documents are in the custody of the courier service. The applicant should note the tracking numbers of all envelopes used and submitted.

e. Ensure that all submitted requirements are correct and in order. Otherwise, incomplete submissions will not be processed and will be returned to the informant at their own expense.

 

Get PSA-authenticated Marriage Certificate Online

You may request PSA-authenticated copy of the ROM after 6 to 12 months from the date of reporting at www.psaserbilis.com.ph. The Consular Officer is authorized by the Philippine Government to solemnize marriages provided it is between one male and one female, both parties are Filipino citizens, and the ceremony will take place within the premises of the Philippine embassy or consulate.

 

Pre-processing by mail/email. Solemnization of marriage by appointment.

 

 

 

Documentary requirements


  • Marriage License application form (Downloadable forms at the bottom of this page)

The bride and groom must each accomplish a Marriage License Application Form. The forms have to be signed in front of a Consular Officer.

  • Birth Certificate

The bride and groom provide one (1) original & three (3) photocopies of their PSA/NSO-authenticated birth certificate.

  • CENOMAR

The bride and groom must each provide one (1) original and three (3) photocopies of their Certificate of Non-Availability of Record of Marriage (CENOMAR) issued by the Philippine Statistics Authority (PSA). A CENOMAR is valid for only six (6) months from the date of issue by PSA.

  • Philippine Passport & proof of Filipino Citizenship

Each party must submit four (4) photocopies of their valid Philippine Passport data page including four (4) photocopies of a proof of their current Philippine citizenship such as: valid US Visa, US Permanent Resident Card or work permit.

  • Photos

Each party must submit two (2) passport-sized colored photos.

  • Fees

Marriage License: US$25.00
Solemnization Fee: US$60.00

 

 

Additional requirements for each personal circumstance

 
  • For a contracting party who is a widow or widower

  • For contracting party who had a previous marriage annulled by court or dissolved by a Philippine court recognition of foreign divorce

Affidavit of parental consent

Parents of a contracting party aged 18-21 years old must execute an Affidavit of Parental Consent. The affidavit must be duly notarized and bear an Apostille.

  • For a contracting party aged 22-25 years old

Affidavit of Parental Advice

Parents of a contracting party aged 21-25 years old must execute an Affidavit of Parental Advice. The affidavit must be duly notarized and bear an Apostille.​​ 

 

STEPS:

2. Submit application for a marriage license and Certificate of Legal Capacity to Contract Marriage (LCCM) at the Consulate

Both parties must be present at the Consulate to apply and submit documentary requirements for application of marriage license and Certificate of Legal Capacity to Contract Marriage (LCCM). Please note that all original PSA documents will be retained by the Consulate and will not be returned.

3. Publication of Marriage Application for 10 Consecutive Days

The application for a marriage license or the marriage banns shall be posted or published for ten (10) consecutive days before the marriage license is issued.​

4. Issuance of LCCM & Marriage License

The Consulate shall issue the LCCM and a marriage license which is valid for a period of 120 days from the date of issuance. The applicants should get married at the Consulate within those 120 days.

5. Marriage Solemnization at the Consulate

The applicants coordinate with the Consular staff on the date and time of marriage solemnization. They should provide information about the names and addresses of their witnesses.​

6. Get PSA-authenticated Marriage Certificate Online

You may request PSA-authenticated copy of the Marriage Certificate after 3 to 6 months from the date of marriage. at www.psaserbilis.com.ph.
